Assignment Request Forms

If you prefer to send your assignment to us, please download the appropriate PDF form and complete as much information as you can.

You will need Adobe Acrobat Reader to view the forms. If you do not have it on your computer you can download it from the Adobe website for free. Click this link to get Adobe Acrobat Reader.

Once the appropriate form has been completed, you can email or fax it to us. To email, click the “email” button in the upper right-hand corner of the form and then select your email method from the pop-up box. (Please be aware that in-browser PDF document readers do not support Adobe PDF form buttons. For the email and print buttons to work, you must download the form and open the file in Adobe Acrobat Reader. See *Note, below, for more info.)

To fax the form, simply print it out and fax to (702) 446-8409. (But, again, for the print button within the form to work, you must open the PDF file in Adobe Acrobat Reader. See *Note, below, for more info.)

A) You first need to determine the case type you have and wish to assign so you can fill out the appropriate assignment request form:

  1. Minor Impact and Soft Tissue Analysis
  2. Automotive Component Failure or Vehicle Fire Loss
  3. General case assignment or Structure Fire

B) Click on the corresponding form link below and save it to your hard drive. (Again, you must download the form to use the email and print buttons within the form. See *Note below.) Fill out the form to your best ability and email or fax to Sage Investigations. If you have photos that need analysis as part of the assignment, do not fax them. Please send via email or regular postal mail; faxes will not provide the detail required.

*Note – browser limitations with PDF forms: Most web browsers (Google Chrome, Firefox, Edge, IE, Opera) now have their own in-browser PDF document viewers. Unfortunately, these built-in PDF viewers do not support Adobe’s ActiveX controls which enable buttons to function, i.e. the email and print buttons within the forms below.

To ensure that the email and print buttons work properly, the form must be downloaded (saved to your hard drive) and then opened in “Adobe Acrobat Reader.” Some browsers like Chrome and Edge provide a “save as” icon in the upper right-hand corner once the document is opened within the browser.

Also, with your mouse positioned over the opened document in the browser viewer, you should be able to right-click and “save as.”

If you use these forms regularly, there are free plugins/extensions for most browsers which will automatically open PDF documents in Adobe Acrobat Reader rather than in an in-browser viewer.

Alternatively, you can permanently disable the in-browser viewer so that PDF documents and fillable PDF forms always download rather than open in your browser’s viewer. The settings vary by browser, of course, so a Google search is your best bet for this approach. For example, to learn how to disable the built-in PDF viewer in Chrome (v60+), click this link.

Form #1: Minor Impact Soft Tissue

This form is for cases involving low impact and bodily injury claims. Sage analysis will determine the speeds and forces and render expert opinions whether or not the impact and the claimed injury is consistent with the human tolerance to injury in minor vehicle collisions.  Please fill out the request form and forward any documentation for this case. Once received, the assigned expert will contact you with a verbal report of their findings and a written report provided with your authorization.

Form #2: Vehicle Component Failures and Vehicle Fires

This request form is for vehicle component failures (i.e. brakes, steering, tires, transmission, etc.) and vehicle fires. Based on location and case type, your case will be assigned an appropriate expert to determine the cause of the particular auto loss. Once the case has been analyzed, the expert will contact you with a verbal report of their findings and a written report can be provided with your authorization.

Form #3: General Assignments / Structure Fires

All other case types can be assigned by filling out this universal assignment request form.  Please fill out this form with as much detail as you can and attach any pertinent documentation. Our office staff will review the request and assign the expert.  The selected expert will contact you to further discuss the case and ensure a complete understanding of the scope of the assignment.
